The M4/16 magazine is not really that weird. That can easily happen when the spring collapses.

I had that happen to my Glock, rounds were rattling around in the mag like a box of smarties.....not good for your duty pistol.

I've never had that happen, with my personal weapons or in the military (infantry). What you need to do is remove the foot plate of the magazine, take the spring out and stretch it. Then put it back in the magazine. It loses tension over time from being fully compressed when loaded with rounds. If you don't stretch the spring, you'll also experience mis-feeds, double-feeds and other stoppages.
