It's always my fear about this hassle when I purchase things from overseas. But so far, I've been able to escape the issues you've had. I purchased a few watches from a couple of businesses in Hong Kong. I think both shipped DHL and they've arrived with no surprises. What I paid online was what I paid total with no extra charges. Shipping was pretty quick too with one shipment which actually arrived the next day which shocked me.
I purchased items from a couple of European retailers and I think those where shipped via UPS. Again no issues.
